About Susan L. Frank
Susan Frank is president and CEO of ASF Solutions LLC, a company supporting healthcare redesign by focusing on Population Health Management and Operational Strategies to optimize the triple aim approach: better outcomes, better patient experience, and improved cost. Susan works with teams to lead Population Health Management (PHM) strategies that increase revenue for facilities or physician organizations, focusing on innovation and clinical, physician, and payer integration. ASF Solutions assists organizations in re-engineering a comprehensive, supportive care delivery system that improves outcomes, gap closure, and wellness services required in the changing culture of healthcare.
In her previous position as Vice-President of Population Health Management and Program Development with Allegheny Health Network (AHN), Susan was responsible for leading system-wide care management and care coordination redesign for seven facilities at AHN. Her responsibilities included oversight, planning, and implementation of initiatives to improve quality of care through improvements in case management, utilization review, and care coordination. Susan had also developed and implemented a Community Care Network and Navigation program for high-risk patients that showed significant savings, improved quality, and patient satisfaction. She also led a system-wide health coach program across six western Pennsylvania universities and colleges that have produced 283 students who supported patient care in the home. Susan has also taught the first Health Coach minor at a University in Pittsburgh.
